VA Form 21-4138 is a general-purpose form used to provide a “Personal Statement.” While medical records show clinical data (like range of motion or blood pressure), this form allows you to describe the functional impact of your disability—the human side of the story that doctors often miss.
Do not simply list your diagnosis. Use this form to describe:
