Step 1: Starting Your Claim
VA Form 21-0966
Name: Intent to File a Claim for Compensation and/or Pension
Purpose: Submitting this form “locks in” your effective date while you gather evidence. You have one year from the date you submit this to file your full claim.
VA Form 21-526EZ
Name: Application for Disability Compensation and Related Compensation Benefits
Purpose: The primary form used to apply for service-connected disability benefits. Use this for new claims or claims for increased ratings.
Step 2: Gathering Evidence & Statements
VA Form 21-4138
Name: Statement in Support of Claim
Purpose: Use this to write your “Personal Statement.” Explain in your own words how your disability affects your work, social life, and daily routine.
VA Form 21-10210
Name: Lay/Witness Statement
Purpose: Used for “Buddy Letters.” Have a spouse, friend, or fellow service member fill this out to describe the symptoms they witness you dealing with.
VA Form 21-4142
Name: Authorization to Disclose Information to the Department of Veterans Affairs
Purpose: Gives the VA legal permission to request your private medical records from non-VA doctors.
Step 3: Filing an Appeal (Decision Reviews)
VA Form 20-0995
Name: Decision Review Request: Supplemental Claim
Purpose: Use this form if you have new and relevant evidence to submit for a previously denied claim.
VA Form 20-0996
Name: Decision Review Request: Higher-Level Review
Purpose: Use this if you believe the VA made a legal error or ignored evidence that was already in your file. You cannot submit new evidence with this form.
VA Form 10182
Name: Decision Review Request: Board Appeal (Notice of Disagreement)
Purpose: Use this to appeal your case directly to a Veterans Law Judge at the Board of Veterans’ Appeals (BVA).
Step 4: Specialized Benefits
VA Form 21-8940
Name: Veteran’s Application for Increased Compensation Based on Unemployability
Purpose: The application for TDIU. Use this if your service-connected disabilities prevent you from keeping a steady job.
VA Form 21-686c
Name: Declaration of Status of Dependents
Purpose: Use this to add a spouse or children to your award (usually requires a rating of 30% or higher) to increase your monthly payment.
VA Form 20-10206
Name: Freedom of Information Act (FOIA) or Privacy Act Request
Purpose: Use this to request a copy of your C-File (Claims File) or Service Treatment Records.
