VA Form 20-0995: Supplemental Claim

What is VA Form 20-0995?

VA Form 20-0995 is the Decision Review Request used to file a Supplemental Claim. This is one of the three appeal lanes under the AMA system. You use this form when you have been denied, but you now have new evidence (that the VA has not seen before) that fixes the reason for the denial.

When should I use this form?

Use Form 20-0995 if:

• You were denied because of a missing Nexus, and you now have a Nexus Letter.
• You were denied for lack of a diagnosis, and you now have recent medical records confirming one.
• You want to reopen an old claim that was denied more than a year ago, and you have new evidence that is relevant to your claim.

What counts as “new and relevant” evidence?

You cannot simply ask the VA to look again. You must attach documents that:

• New: The VA has never reviewed this document before.
• Relevant: The document proves or disproves a vital fact (e.g., if they denied you for “no in-service event,” submitting a buddy letter witnessing the event is relevant).

Timeline

You generally have one year from the date of your denial letter to file this form and preserve your back pay. If you file after one year, your effective date will reset to the date you file this form.
